Annex A: How to complete the application form

Applying in Prince Edward Island

Eligible employers in the province of Prince Edward Island can apply for the program through their provincial government. Employers in all other provinces and territories can follow the application steps below.

Application deadline

Applications to the program will be accepted until August 31, 2020. The application deadline may be changed in the event that the mandatory 14-day self isolation requirements imposed under Quarantine Act are lifted or extended.

Eligibility criteria

Before you begin your application form, decide if you meet the eligibility criteria detailed in Section 2 of the Mandatory Isolation Support for Temporary Foreign Workers Program Applicant guide.

Application form instructions

PDF forms

If you have problems opening a PDF form in your Internet browser, you need to:

  • Use your computer. The forms may not open on mobile devices (iPads, tablets, mobile phones).
  • Make sure you have Adobe Reader 11 (or higher) already installed on your computer. If not, download Adobe Reader 11 or higher for free. Adobe has help for solving common issues.
  • You must save the file on your computer in a place you can remember.
  • Open the file on your computer using Adobe Reader 11 or higher.
  • If you can’t open the form with Adobe Reader 11 or higher, contact the program.
How to download and open a PDF form
  • Microsoft Windows Operating System computers using “Internet Explorer” or “Microsoft Edge” as your browser
    1. Right-click on the link you want to save
    2. Select “Save target as” or “Save link as”
    3. Choose the location on your computer where you would like to save the file
    4. Select “Save”
    5. Open Adobe Reader 11 or higher
    6. Select “File”
    7. Select “Open”
    8. Go to the location where you saved the file
    9. Select the PDF file
    10. Select “Open”
  • Microsoft Windows Operating System or Apple computers using “Chrome” or Firefox” as your browser
    1. Right-click on the link you want to save
    2. When the option menu appears, choose "Download link file" or "Download link file as"
    3. Choose the location on your computer where you would like to save the file. Your computer will start the download once you have selected a location.
    4. Open Adobe Reader 11 or higher
    5. Go to the location where you saved the file
    6. Select the PDF file
    7. Select “Open”

Form instructions

  • All mandatory fields must be completed and all questions must be answered in order for the form to be considered complete.
  • Ensure your application can be processed as quickly as possible
    • fill in the form electronically (enter your answers into the fields)
    • email the form to Agriculture and Agri-Food Canada
  • Hand written and/or mailed or faxed forms will take longer to process.
  • Only one (1) application can be submitted per applicant.
  • If changes are required to an already submitted application, or you wish to withdraw an application, do not resubmit a new application form, please contact the program.
  • Applicants will receive an acknowledgement within three (3) business days for email applications. If you choose to submit your application form (PDF) by mail or fax, it could take longer to receive your confirmation.

Section 1: Applicant Information

Applicant type, name, mailing address and email address

Select the applicable Applicant Type. The Applicant Type will affect some of the selections available in other fields.

Name and address of signing authority

Enter the name and address of signing authority if different from applicant name and address above. Use the checkbox to use the same address as the Applicant.

Additional Applicant information

Enter the Business Number or Trust Number.

The Business Number is mandatory for all applicants other than Trusts. Your application cannot be processed without it.

Note: when entering the Business Number, ensure to enter all 15 characters of the Business Number. The form will not accept invalid Business Number formats.

Enter the Applicant Industry group using the drop down options.

Section 2: Project Details

Provide responses to all questions in this section.

Eligible Activities

Select all incremental costs that you incurred associated with the temporary foreign worker’s mandatory 14-day isolation period.

Temporary Foreign Worker Information

Enter the total number of temporary foreign workers hired or who will be hired between March 26, 2020 and August 31, 2020.

The maximum MISTFWP contribution will be automatically calculated. If the anticipated expenses related to the mandatory 14-day isolation of temporary foreign workers will be less than the maximum contribution calculated, enter the anticipated expenses.

Section 3: Other Project Information

Enter any unpaid debts to the Government of Canada in this section. If there are no unpaid debts to the Government of Canada, select no.

Enter details of any paid lobbyists working on the applicant’s behalf in this section. If there are no lobbyists working on the applicant’s behalf, select no.

Voluntary information

This section is voluntary and will not impact the processing of the application.

Section 4: Declaration and Signature Page

Please read the declaration and check the three (3) mandatory declaration checkboxes. The applicant should complete the name and date fields.

Once the application has been completed, select the submit button. An email should automatically generate with the program’s email address and the application as an attachment. If the email does not automatically generate, please save the form on your computer and email the completed form to aafc.MISP-PAIO.aac@canada.ca.
