Part 6 - Privacy and Certification Clause and Signature

Agriculture and Agri-Food Canada (AAFC) is committed to protecting the privacy of your information. The information on this form, including a Participant Identification Number (PIN) and Social Insurance Number (SIN), is collected under the authority of Section 4 of the Farm Income Protection Act (FIPA) and will be used by the Administration exclusively for the purposes of processing your AgriInvest Estate Administration Form. You have the right to request access to your personal information held by AAFC and to request changes to incorrect personal information. For more information about your rights under the Privacy Act, contact the AAFC Access to Information Privacy Coordinator at aafc.atip-aiprp.aac@canada.ca and reference AAFC PPU 183. By submitting this form, you:

  1. Certify that the information provided is complete and correct and
  2. Understand it is a criminal offense to make a false statement and that any declarations made are subject to audit.

Certified Copies

Do not mail your original documents to us. We require certified copies to ensure documents are authentic. A certified copy is a photocopy of an original document that has been certified as being a true and accurate copy of the original document. If an original document has information on both sides, be sure to certify both sides. Neither you nor any person related to you can certify the documents.

People who certify copies must compare the original document to the copy, state their official position or title, sign and print their name, give their telephone number and the date they certified the document. They must also write the following statement on the copy:

If your copies are missing any of the above elements, we cannot accept them and you will have to submit new, properly certified copies. This will result in delays processing your AgriInvest Estate Administration Form.
